Filling this out in advance will save us time in session.

If your child (anyone under the age of 18) is going to involved in therapy, the informed consent form for minors may be also downloaded.

 

If you would like to fill out a release so I may speak to a previous therapist, or your doctor, you may download my Release Form.

 

Payment: My fee for an individual, couple or family session is $90 for a 50-minute session.  I accept cash or check. 

 

I am a participating provider with the following insurance companies:

 

Aetna -Behavioral Health Program and their Employee Assistance Program (EAP) 

BlueShield- Behavioral Health and EAP

Magellan- EAP

To obtain authorization for visits please call the number on the back of your Health Insurance ID card, or contact your HR rep to obtain the number to your EAP provider.

 

If you do not have insurance through one of these companies, it is possible that you may have Out of Network benefits that could cover part of the session.  To find this out please contact your insurance company.

The entrance to the office complex is located on Wildridge.  As you enter, go to the end of the complex.  Building H is the last building in the complex and suite H-1 is the first office in Building H.  There is a waiting room as you enter the door to the office suite.
